THE ONLY WARRANTIES, EXPRESSED OR IMPLIED, GRANTED IN CONNECTION WITH THE PRODUCTS SOLD WITH THIS FUNERAL SERVICE, ARE THE EXPRESSED WRITTEN WARRANTIES, IF ANY, EXTENDED BY THE MANUFACTURER THEREOF. THE JOHNSON FUNERAL SERVICE, INC. HEREBY EXPRESSLY DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, RELATING TO ALL SUCH PRODUCTS, INCLUDING, BUT NOT LIMITED TO, THE IMPLIED WARRANTIES OF MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE.

CERTAIN FUNERAL SERVICES MAY BE PROVIDED OFF PREMISES BY OTHER FUNERAL SERVICE PROVIDERS.


The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

This list does not include prices for certain items that you may ask us to buy for you such as cemetery or crematory services, flowers and newspaper notices. The prices for these items will be shown on your bill or the statement describing the funeral goods and services you selected.

 


 

PROFESSIONAL SERVICES

 

Professional Services of Funeral Director and Staff - $2250.00

 

This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

Our fee for the services of funeral director and staff includes, but is not limited to:

  • Staff to respond to initial request for service: arrangement conference with family or responsible party
  • Arrangement of funeral
  • Preparation and filing of necessary authorization and permits
  • Recording vital statistics
  • Preparation and placement of obituary notices
  • Staff assistance prior to, and following the funeral, including coordination with those providing other portions of the funeral, e.g. cemetery, crematory and others.

Also included in this charge are overhead expenses relative to our facility such as:

  • Insurance
  • Maintenance and utility expenses
  • Secretarial and administrative costs
  • Equipment and inventory expense

 

Embalming - $ 850.00

 

Embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

 

Other Preparation of Body

Staff & Equipment for Services

 

  1. Staff services for visitation at the funeral home or at another location - $ 200.00
    Our services include:
    • Set-up of visitation area
    • Placement of encased remains
    • Display of floral arrangements
    • Supervision of and attendance during the visitation
  2. Staff services for funeral service at the funeral home or at another location - $ 400.00
    Our services include:
    • Coordination the funeral arrangements
    • Supervision of funeral
    • Staff to attend funeral ceremony
  3. Staff services for memorial service at the funeral home or at another location - $ 400.00
    Our services include:
    • Coordinating the memorial service arrangements
    • Supervision of the memorial service
    • Staff to attend service
  4. Staff services and equipment for graveside service - $ 300.00
    Our services include:
    • Accompaniment of remains to cemetery
    • Supervision of graveside service
    • Staff to attend service
  5. Administrative Fee For Anatomical Donations - $ 350.00

 

Transfer of remains to Funeral Home - $ 350.00

 

Within a 25 mile radius of funeral home. Over 25 miles add $2.25 per loaded mile.

LIMITED SERVICE PACKAGES

 

Forwarding Remains to another Funeral Home - $ 3450.00

 

This charge includes:

  • Professional Services of Funeral Director and Staff
  • Transfer of Remains to Funeral Home
  • Embalming

(No viewing, funeral service, merchandise or hearse for transportation to airport is included above.)

 

Receiving remains from another funeralhome - $ 2600.00

 

This charge includes:

  • Professional Services of Funeral Director and Staff
  • Funeral Coach for transportation of remains to cemetery

(Above does not include transportation from airport, viewing/visitation, staff services for ceremony, or funeral home equipment for church services, flower van or service/lead car.)

 

Direct Cremation - (Ranges from $ 2850.00 to $ 9450.00)

 

Our charge for a direct cremation includes Professional Services of Funeral Director and Staff, Transfer of Remains to Funeral Home, Handling of unembalmed remains, Refrigeration, Crematory Fee, alternative container and Medical Examiner Fee of $ 50.00.

State and local laws do not require a casket for direct cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are cremation cardboard trays and hardboard trays.

The charges listed above do not include any attendant rites, visitation/viewing or ceremony.

  • Direct cremation with container provided by the purchaser - $ 2850.00
  • Direct cremation with Minimum alternative container - $ 2850.00
  • Direct cremation with Standard Brown - $ 3150.00
  • Direct cremation with Shaker Pine Cremation Casket - $ 3850.00

 

Immediate Burials - (Ranges from $ 3500.00 to $ 12500.00)

 

Our charge for an immediate burial, without any attendant rites or ceremonies, includes Professional Services of Funeral Director and Staff, Transfer of Remains to Funeral Home, Handling of unembalmed remains, Refrigeration, Funeral Coach for transportation to the cemetery and Graveside service. (This does not include cash advance items such as opening/closing of the grave, cemetery charges, newspaper notices, death certificates or special charges for rock.)

  • Immediate burial with container provided by purchaser - $ 3500.00
  • Immediate burial with the Gemini or Bradford casket - $ 4495.00

 

Anatomical Donations - $ 850.00

 

Our charge for anatomical donations, without any attendant rites or ceremonies, includes:

  • Administrative Fee
  • Transfer of Remains to Funeral Home
  • Handling of unembalmed remains
  • Refrigeration

Terms & Method of Payment

 

TERMS OF PAYMENT:

 

The total funeral account is due and payable no later than 30 days from arrangement date. If payment is not made in full by that date, I/we understand that I/we will be in default of this agreement and that a late fee or penalty of 1 1/2 % per month (18% APR) will be added to the unpaid portion of the balance due for each month any balance remains unpaid. I/we agree to pay and /or reasonable attorney’s fees and court costs. I/we agree that the liability is being personally assumed by me/us and is in addition to the liability imposed by law upon the estate, and this agreement does not constitute a release of liability by my/our signature below. I/we waive my/our Homestead Exemption with regard to this debt. Acknowledgment and agreement of the above is hereby made.

 

Method of Payment:

 

  • Cash:

    Payment received for funeral account in 30 days or less is considered cash at time of arrangement.

  • Life Insurance:

    If the family wants life insurance proceeds to be the payment method for the funeral expenses, two things must be considered. First is the insurance in force at the time of death and is the beneficiary willing for the proceeds to be used for funeral payment. Normally the life insurance proceeds can be received in 30 days.

  • Credit Card:

    For the family convenience, we have started accepting credit-card (MasterCharge or VISA) as a method of payment.

  • Pre-Funded Funeral Contracts:

    The proceeds from all pre-funded funeral arrangements will be accepted as a cash payment.

  • Cash Advance Items:

    Cash advance items listed on the funeral agreement, are items that other vendor (not the funeral home) charges. It may be either services or merchandise items, such as grave open/closing fees, newspaper charges, flowers, etc. These items the vendors want their fees by the time of the service. The family may make a check for each vendor or combine all in one, and we will pay each for you.
